Instructions on Working With Forms

Important! Please be sure to read Guidance Notes for each form if available.

Don't store copies of these forms on your own hard drive for future use! Always come back to this download page for forms as needed, to ensure that you are using the current version.


Types of Downloadable Forms

  • Forms in Microsoft Word Format: Many UBC forms are available in Microsoft Word format as template (*.dot) files. These can be filled out on your computer then printed, circulated for signatures, and sent to the appropriate office. They are compatible with Word for Windows 6.0 and later versions running on a PC, and should also work for users of newer versions of Word for Mac and Wordperfect. Office of Research Services only provides support for users of Microsoft Word for Windows. These templates may not work for Word 95 and earlier versions of Word for Windows and earlier versions of Word for Mac. If the templates do not work for your computer, you may need to download the pdf version of the form as an alternative. The 'guidelines' or 'guidance notes' are Word document files (*.doc). Tips for using MS Word Template forms can be found below.

  • Forms in Adobe Acrobat (PDF) format: In some cases forms are only available in pdf format. Also, if your computer does not support MS Word template files properly (e.g., some older Macintosh computers) you may need to use the pdf alternative. Unfortunately, at present our pdf forms cannot be filled in on your computer. You will need to print out the pdf form and fill it in manually (e.g., with typewriter). Tips for using Adobe PDF forms can be found below.

  • JetForm Filler Forms: Research Services no longer supports JetForm Filler software and all UBC forms in the JetForm Filler format are now outdated. Please use MS Word forms or pdf forms as appropriate.


Tips for Working with MS Word Template Forms

  • Using the Forms
    • Before beginning, print a hard copy of the guidance file (if available) for reference
    • Open the template (*.dot file).
    • Enter data into the shaded fields in the form
    • Use TAB to move from one field to another; SHIFT-TAB to go in reverse
    • Text may be pasted from other applications
    • The amount of text that can be entered in each box is limited and cannot be changed by the user; the formatting can't be altered (however, see the next section)
  • Saving the Completed Form
    • Assign a name to the completed form
    • Save as a Word Document (*.doc file)
    • Do not overwrite the Template (*.dot file) with your completed form
    • If you do overwrite the template, you can download a new copy from this web site


Tips for Working With Adobe PDF Forms

  • What is a PDF file?
    A Portable Document Format (pdf) file is a self-contained cross-platform document. In plain language, it is a file that will look the same on the screen and in print, regardless of what kind of computer or printer someone is using and regardless of what software package was originally used to create it.
  • Can I fill out the PDF forms on my computer?
    No, at present these are forms which you can only print out and then fill in (e.g. with typewriter). They are primarily intended for those who cannot use Microsoft Word template forms (e.g., users with older Macintosh comuputers).
  • Can my computer open PDF files?
    Virtually anyone - including Windows, DOS, Macintosh, SUN, and UNIX system users - can view and print a PDF file using the free Adobe Acrobat Reader from the Adobe web site. If your computer doesn't already have this software you must download and install it (see below.)
