Signature Forms
In most cases, the ORS Signature Form can be downloaded and used to collect signatures for your proposal if signature pages are not provided in the proposal package. Download the ORS Signature Form.
However, some faculties require the use of a specific cover sheet:
The Dean’s signature is not required on proposals originating in the Faculty of Applied Science, Faculty of Science, or School of Kinesiology. See agency-specific requirements for competition-specific details.
